I am responsible for the return of Digital Terminal(s), remote(s), modem(s), and associated equipment. If equipment is not returned in good working condition to the AWN office within five (5) business days of termination, my account will be charged up to $500 for Digital Terminals, $15 for each remote, and up to $150 for the modem(s). I agree that AWN may use Customer Proprietary Network Information (CPNI) to market additional services – this information will not be disclosed to any third party. Program Terms and Conditions will be issued with service equipment.
